certificate for chronic diseases for opd treatment

Sir,

The FCI retired employees are entitled for Rs.12000/- per financial year for OPD treatment( in addition to IPD) for 13 chronic diseases. I would like to know who can issue this certificate for charonic diseases . Please clarify whether the hospitals emlpannelled by the FCI can issue such certificate to retired employees including class I, II , IIi employees for this purpose of reimbursement of OPD treatment charges or name other authority who are authorise to issue certificate.
